Many people take multiple medicines that are prescribed by more than one doctor. This increases the risks of drug interactions, side effects and other complications. Our pharmacists are medication experts and can help you better manage your medications, prevent problems from medications interacting and help you solve or reduce side effects from a medication.
Medication therapy management (MTM) services are offered by health insurance companies to help members get the best results from their medicines. If you are eligible for Medication Therapy Management (MTM) services through your health plan, your local pharmacist may talk with you about your medications. You may also receive a phone call from one of our MTM pharmacists inviting you to review one or more of your medications. For questions about your eligibility, you may contact your health plan about the MTM program.
Your local pharmacist may talk with you about your medications. If you take a lot of medications, your pharmacist may even ask you to come in to review all your medications. This review doesn’t cost anything extra if you’re eligible for MTM services through your health plan – it’s there to help you get the best results from your medications. Ask your pharmacist if they provide MTM, or Medication Therapy Management, services with OutcomesMTM®.
You may receive a phone call inviting you to review one or more of your medications with one of our MTM pharmacists. In most cases, we call from (515) 974-0785, but, for some health plans, we may call from a local number. If you need to call our MTM pharmacist back, you can reach them on our toll-free line, (855) 795-6337. Talking to our pharmacist doesn’t cost anything extra if you’re eligible for a medication review through your health plan – it’s there to help you get the best results from your medications.
I already see my doctor and local pharmacist. How will I benefit from talking with you?
Patients who understand their medicines get better results. We can offer suggestions to help with side effects, lower medication costs and remembering to take your medicines. This is also a great time to ask any questions you have about your medicines. Pharmacists work with other members of your healthcare team as needed.
Can you explain the pricing of my medicines?
We work with your health insurance to provide members with medication therapy management services. We do not have access to prices for your medicines, but we can usually recommend other medicines or generics that may help to lower the cost. For specific pricing information, please call the phone number on the back of your insurance card.
Why do you need my personal information?
Privacy is very important to us. Before we talk about your protected health information, we want to make sure we are speaking with the correct person.
What services do you provide?
Our pharmacists work with you and your doctor to solve medicine-related concerns.
- Comprehensive Medication Review (CMR): Our pharmacist will talk with you about your prescriptions, vitamins, herbals, supplements and other over-the-counter medications. Our pharmacist looks for potential medication conflicts, duplications, side-effects or ways to save you money. You can ask any questions you have about your medicines. After the review, our pharmacist will work with you, and your doctor if necessary, to resolve any medicine-related concerns. We will mail you a medication list that you can share with your doctor.
- Targeted Medication Review: We may call to check in on a specific medication to make sure it is working well for you or to talk about a medicine-related concern. This is a good time to ask any questions you have about the medicine. If you have trouble remembering to take your medicine, our pharmacist may be able to suggest ways to help you stay on track. If there are concerns about the dose or safety of a medicine, our pharmacist will ask your permission to talk with your doctor to find the best solution for you.
