UCare Offers Medication Therapy Management Program with OutcomesMTM® - 05.06.2016
UCare, an independent, not-for-profit health plan based in Minnesota, is now offering medication therapy management (MTM) services through the OutcomesMTM Personal Pharmacist™ Network to qualifying members of its Medicare plans.
The OutcomesMTM network enables eligible UCare members to receive personal medication review services from local, specially trained pharmacists in their community. The program became active Jan. 1, 2016, for members of UCare for Seniors Medicare Advantage plans, UCare's Minnesota Senior Health Options (UCare's MSHO) and EssentiaCare Medicare plan, which is a Preferred Provider Organization (PPO) partnership with Essentia Health.
"We are excited to offer our Medicare members an enhanced medication therapy management program," said Kerry Bendel, RPh, Director of Pharmacy, UCare. "Pharmacists play a key role in helping patients get the most benefit from their medications. Receiving a personalized medication review from a local pharmacist can lead to better health and decreased medical costs."
UCare Medicare members who meet MTM program criteria can receive an annual comprehensive medication review (CMR). Members meet face-to-face with a pharmacist to review their current medications and address possible conflicts, duplications or other barriers to medication treatment. Pharmacists may also address specific medication opportunities focused on improving adherence, safety and cost effectiveness.

- National Committee for Quality Assurance (NCQA)-accredited plans for Minnesotans shopping on MNsure.

- Medicare plans with an "Excellent" NCQA rating, and a new partnership with Essentia Health in northern Minnesota.
- Programs for older Minnesotans with Medical Assistance, or with both Medical Assistance and Medicare.
